I acknowledge by signing this consent form that I have been given full opportunity to ask, any questions which I may have about obtaining the permanent makeup treatment from Kenosis Beauty and that all my questions have been answered to my full satisfaction. I specifically acknowledge that I have been advised of the facts and matters set forth below, and agree to the following:

I acknowledge that it is not reasonably possible for representatives and employees of Kenosis Beauty to determine that I might have an allergic reaction to the pigments or process used during my permanent make-up and I agree to accept the risk that such a reaction is possible.

I acknowledge that infection is always possible as a result of me not taking proper care of the permanent make-up while it is healing, and that any retouch work needed due to my own negligence will be done at my own expense.

I acknowledge that the obtaining of my permanent makeup is by my choice alone. I consent to the application of the permanent makeup treatment. I also take into consideration that any action or conduct of the employees of Kenosis Beauty is reasonably necessary to perform the permanent makeup procedure.

I agree to release and forever discharge and hold harmless Kenosis Beauty and its employees from any and all claims and legal actions arising from or connected in any way with the permanent makeup treatment or the procedures and conduct used to apply my permanent makeup.
